Thanks @Hovercraft. I needed to push the adapter down onto the face of the Lyra strongly in order for all of the touchplates to respond.
I wouldn't say the adapter was machined incorrectly.

Has anybody here received a Lyra adapter that doesn't seem to fit quite right? Mine won't sit on the far ends of the Lyra at the same time - it's either left channels or the right, but not both. After shipping out from SOMA towards end half of October, the unit I ordered arrived safe and sound today.
For those folks who are still waiting, it took about 3 months from ship to arrival in northeast, US.
