Thanks @Hovercraft. I needed to push the adapter down onto the face of the Lyra strongly in order for all of the touchplates to respond.
I wouldn't say the adapter was machined incorrectly.

Has anybody here received a Lyra adapter that doesn't seem to fit quite right? Mine won't sit on the far ends of the Lyra at the same time - it's either left channels or the right, but not both. Accordingly, playing the pads on the adapter won't engage the lyra, whereas the pads played directly on l...

For those folks who are still waiting, it took about 3 months from ship to arrival in northeast, US.
